Moderate LNAn increase in serum creatinine of 0.2–1.0 mg/dl if baseline is <2 mg/dl or 0.4–1.5 mg/dl if baseline is >2 mg/dl or an increase in urine Pr/Cr to ≥1 if baseline is <0.5 or ≥2 if baseline is 0.5–1 or an increase of ≥2-fold with an absolute Pr/Cr <5 if baseline is >1
